Billing and Insurance

It is the responsibility of the patient to know and understand the benefits associated with their own insurance. If a patient has a deductible that has not been met, or a claim is denied due to limitations in coverage, the patient will be responsible for the remaining balance of their claim. The patient’s insurance must be active at the time of the visit. The patient will be responsible for the total amount of the visit if a claim is denied due to lapse in coverage. At the time of your visit, payment of applicable co-insurance, co-pays, or deductibles is expected.

Out of state/area benefit coverage will not be covered in certain plans. Please check with your insurance company to make sure your visit will be covered.

Medicare

We accept Medicare and supplemental plans.

We do not accept any HMO plans or Medicaid.

Are you currently uninsured? Ask us about our affordable self-pay rates.

Billing

The patient will be expected to pay for any applicable co-insurance, co-pays, or deductibles at the time of the visit.
